What we collect
The diagnostic intake form collects contact details, organizational information, and a description of the situation you want reviewed, including several structured ratings used to route your request internally.
How it is used
INVARYX reviews submitted information to determine whether there may be a fit for a potential advisory or diagnostic engagement. Submissions are reviewed by INVARYX staff and are not shared publicly or with third parties for marketing purposes.
What not to submit
Please do not submit legally privileged, confidential third-party, classified, or highly sensitive regulated information through this form. If your situation requires that level of confidentiality, wait until a direct engagement is underway before sharing it.
Data retention
Submitted diagnostic requests, along with the internal review notes and status history attached to them, are retained for as long as needed to evaluate the request and, if an engagement follows, to support that engagement. Requests that do not proceed to an engagement are retained for up to 24 months from submission to allow for follow-up, after which they are eligible for deletion on request.
You may request that your submission be deleted at any time by contacting INVARYX using the contact details provided in your confirmation email.
Security
Submissions are transmitted over an encrypted connection and stored in a database that only authenticated INVARYX staff can access. Access to the internal review dashboard is restricted by role, and every status change, note, and scoring decision is logged.
